We were asked in early 2023 whether our complex care management platform could support in the care of a 7-year-old boy at home. The child had been born with Hirschsprung syndrome and suffered a subsequent brain trauma, leading to the requirement for 24-hour, 2-to-1 domiciliary care, with the family being actively involved in delivering care.

This individual has an unusually complex care package, with many different physical and behavioural conditions to be managed. Care is delivered and reported by 25 different stakeholders, including agency staff, making around 100 reports per day across different care settings.

Prior to Total Care Manager, the care was managed entirely using paper and Excel. Total Care Manager quickly absorbed the existing care plan, and trained each member of the care team on our system in about 45 minutes per person.

We also carried out some customisation in order to accommodate the service user’s specific requirements around fluid output. Thanks to our highly flexible system and our specialism in hospital-grade reporting, we were able to carry out this customisation work quickly and at no cost to the customer. This included customising our handover reports to ensure no data points were missing after each care shift, and creating warnings for high stoma output.

Following this customisation work, we have captured:

  • 900+ stoma output reports in a year
  • 130 orange warnings for high stoma output, and 9 red warnings

…and we continue to co-develop our solution with the care team as the relationship continues.
