Total Care Manager was founded on our first-hand experience complex care.
CEO Antoine Tooley saw his family life change as a young teenager, when his older brother Tom suffered a traumatic brain injury.
From that day, the family home became a care setting, and some members of the family became care professionals, employed directly by Tom’s care budget to support his care and quality of life.
Problem: electronic health records (EHR) are severely lacking in complex care
Today, electronic health records (EHRs) are seen as a crucial component of effective care delivery. They provide a central collaboration point for the care team, ease management, and in more modern systems, they also provide visibility and control of the data.
The majority of EHR systems currently in use are 20+ years old. Adapted from workforce management platforms, they were only ever really intended for dementia care – where basic care needs are delivered in the last few years of a person’s life.
They were never intended for complex care cases such as brain trauma, birth trauma and other lifelong chronic conditions. This is the provision of hospital-level nursing care, with high degrees of clinical risk, and also a need to support a person’s quality of life as well as their physical wellbeing.
As a result, complex care providers often rely on paper notes, Excel and other data silos for care planning and reporting.
This adds time, cost and complexity, and puts patients at risk. And for families like Antoine’s, it takes over the family home, and impacts on the time that people can spend with their loved ones.
