This team has been working for over 15 years to support an adult who suffered a brain injury in his mid-teens.

For this bright and sociable person, the priority has always been living at home with his family, some of whom work full-time as members of the care team. But with the complexity of his needs, it has sometimes been unclear whether he would be able to continue to live at home.

The service user breathes on a ventilator the majority of the day, feeding and taking medicines via a percutaneous endoscopic jejunostomy (PEJ). Fluid balance, oxygen saturation and temperature are monitored regularly and recorded in the platform, in order to identify potentially dangerous health conditions before they become a problem. Bowel routines are also closely monitored to reduce the risk of accidents and help carers to know when to offer medication.

The system automatically adds periodical and routine equipment changes to carers’ daily tasks, to ensure they are never forgotten about. This includes the monthly tracheostomy change which is reported in the platform.

Aside from his physical needs, this service user is a talented artist with interests in film and literature, and the team helps to support his interests as an integral part of the care plan.

Before Total Care Manager, the team supported by Community Case Management Services was relying on a combination of paper and Microsoft Excel for all care reporting. Jim Ward who works as a Team Leader said:
